These plans provide affordable care through a network of highly qualified dentists. There are no waiting periods, calendar year maximums, or deductibles and no claim forms.

When you need eye care services, you choose whether or not to use a member of UnitedHealthcare's national network of plan providers. You receive the highest benefit (most value) from the plan when you use in-network providers.
